FOR KIDS' KARNIVAL™ PROGRAM . . .

We, the above named group, understand that the company, upon written acceptance of this agreement agrees to supply our group with the following:

• Provide Prize Packs ordered and have an additional case Prize Packs available for the group to purchase in the event more participants show up to the event than planned.

• Provide Games ordered and confirmed by the company.

• Deliver items ordered to event site.

We, the above named group, agree to the following:

• To pay for all charges the day of the event.

• To provide tables for all table games.

• To have at least one adult available to man each game at all times.

• To hold our event indoors to prevent damage to equipment.

• To pay 100% of the replacement cost of any equipment damaged by event participants.

• To pay a $500.00 cancellation fee should we cancel this agreement within 60 days of the date of our event.
